Responsibilities:
- Provide direct patient care, including bathing, dressing, and grooming
- Assist with daily living activities, such as toileting and transferring
- Observe and report changes in patient condition to nursing staff
- Maintain accurate and timely documentation of patient information
- Work collaboratively with the healthcare team to develop and implement individualized care plans
Requirements:
To be successful in this role, you must have:
- A high school diploma or equivalent
- 18 years of age or older
- A valid CNA certificate and current status on the MN CNA Registry
- At least 1 year of experience working in a nursing home or assisted living facility
- The ability to pass a criminal background check
- Excellent communication and interpersonal skills
