Home Health Scheduler (Clinical & Caregiver Coordination)

Position Summary

  • On-site 3 days / week (9am - 5pm)
  • $18.00 / hr - $20.00 / hr (Negotiable Depending on Experience)
  • The Home Health Scheduler is an operations leadership role responsible for coordinating caregiver and clinical staff schedules (including RNs and therapists) to ensure the timely, compliant, and efficient delivery of Interim Healthcare’s home health services. This role upholds strict adherence to AZDHS regulations, payer authorization requirements, and visit scheduling timeframes to support uninterrupted patient care.

Key Responsibilities

Regulatory & Compliance Scheduling

  • Ensure all scheduling activities comply with AZDHS home health regulations, payer requirements, and agency policies
  • Maintain compliance with visit frequency requirements, care plans, and documentation standards
  • Ensure all scheduled services align with physician orders and regulatory requirements

Referral & Care Initiation

  • Receive and manage client referrals, ensuring accurate intake and timely care coordination
  • Ensure start-of-care (SOC) visits are scheduled within required payer and regulatory timeframes
  • Monitor authorization status and proactively manage reauthorizations to prevent lapses in care
  • Coordinate with clinical leadership to prioritize urgent or high-acuity cases

Scheduling Operations

  • Schedule caregivers, RNs, and therapists based on skill set, availability, and patient needs
  • Manage daily scheduling execution, ensuring all visits are fully staffed and completed
  • Handle call-offs, urgent replacements, and real-time schedule adjustments
  • Collaborate with clinical staff to ensure alignment between care plans and scheduling

Communication & Coordination

  • Serve as a central coordination point between caregivers, clinical staff, and office teams
  • Communicate schedule changes promptly and clearly to all stakeholders
  • Monitor daily census, visit utilization, and staffing efficiency
  • Maintain accurate scheduling records in EMR and scheduling systems

Customer Service & Experience Excellence

  • Provide outstanding service to patients, families, clinical providers, payers, and community partners in every interaction
  • Ensure communication is timely, clear, respectful, and solution-oriented
  • Build trust by proactively addressing scheduling concerns and ensuring continuity of care
  • Leave every interaction improved—ensuring individuals feel supported, informed, and confident in their care
  • Reinforce a culture where service excellence is embedded in every scheduling decision and communication

Qualifications

  • 2 years of experience in home health scheduling, clinical coordination, or healthcare operations
  • Strong understanding of AZDHS home health regulations and payer requirements
  • Knowledge of clinical workflows (RN, LPN, therapy services) strongly preferred
  • Experience with scheduling systems and EMRs
  • Strong organizational, communication, and problem-solving skills
