Responsibilities
- Manage and coordinate hospice patient care
- Complete comprehensive patient assessments
- Develop and update individualized plans of care
- Educate and support patients and their families
- Collaborate with physicians and the interdisciplinary hospice team
- Maintain accurate and timely clinical documentation
- Participate in on-call rotation as assigned
Qualifications
- Graduate of an accredited school of nursing
- Current, active RN license in Kansas and Missouri, a Multistate compact license, or the ability to obtain the required license(s) within 30 days of hire
- License must be in good standing
- At least 1 year (12 months) of recent Registered Nurse experience
- Hospice, home health, palliative care, oncology, or medical-surgical experience preferred
- Valid driver's license, reliable transportation, and current auto insurance
- Strong assessment, communication, and organizational skills
- A genuine passion for hospice care and serving patients with compassion and dignity
