We are currently seeking a Full-Time Home Health LPN to join our growing clinical team.

Responsibilities
- Provide skilled nursing care in patients’ homes under the direction of an RN/physician
- Monitor and document patient condition and response to treatment
- Administer medications and treatments as ordered
- Educate patients and families on care plans and disease management
- Assist with coordination of care among interdisciplinary team members
- Complete timely and accurate clinical documentation
Qualifications
- Active Oklahoma LPN License
- Current BLS/CPR Certification
- Valid Driver’s License and Reliable Transportation
- Home Health Experience Preferred
- Strong Clinical Assessment and Communication Skills
- Ability to work independently in a home setting
