Job Duties:
- Provide nursing care to patients in skilled nursing and assisted living facilities
- Administer medications and treatments as prescribed
- Monitor patient conditions and report changes to RN or provider
- Document care accurately and thoroughly in patient records
- Collaborate with care teams to deliver quality outcomes and a positive patient experience
- Ensure a safe and supportive environment for residents and staff
Requirements:
- Active LPN license in the state of Wisconsin
- Minimum of one year of LPN experience in a healthcare facility
- CPR certification (American Heart Association or American Red Cross)
- Strong clinical, communication, and critical thinking skills
- Reliable, professional, and able to adapt in different care settings
