On-Call Triage Visit RN - Part-time Weekends

On Call Triage Visit Registered Nurse - Home Care 

Hours:   Part-time, Weekend Days - We are looking for an RN to be available every other weekend to take calls and triage patients, but also be available to make a visit if there is a need to the patients home.  You would work remotely and go on visits if/when applicable.  The general hours of this position would be 8:00am - 5:00pm Saturday and Sunday, every-other weekend.

Some functions of what you would do as an On Call Triage Visit RN:

• Will take calls per communication from the On Call Service and address them based on type of calls (i.e. Intake/Referrals, Scheduling, Patient Concern, etc.).
• Provides telephone triage and makes patient visits, as appropriate.
• Completes accurate and timely documentation of patient/family services provided.
• Provides education/instructions to the patient/family in accordance with the plan of care.
• Administers medications and treatments as prescribed by the physician and in accordance with nursing standards of practice.

A few must haves for this position:

• Graduate of an accredited school of nursing and licensed to practice as an RN in the state of MN. License is active and in good standing.
• Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years.
• Able to travel within a geographic area serviced by the office. Valid drivers license, reliable transportation, and proof of auto insurance is required.
• Current CPR Certification required.
