As a Home Health Assessment Nurse you will:
- Travel to patients' homes to do Home Care Assessments
- Evaluate and assess things such as mental health, vitals, mobility, ect.
- Provide assessment documentation with accuracy and clarity
- Ensure compliance with nursing standards and regulations
A few must haves for the Home Health Assessment RN:
- Active Registered Nurse (RN) licensed within the state of Indiana
- At least 1 year of recent Home Healthcare experience, as required by facility
- Must be able to work independently, yet together as part of a team
- Must be able to remain calm under pressure
- Proficient at taking vital signs and administering medication accordingly
- Active BLS, physical, TB test, and other medical compliance as required by facility
