If you are team oriented, personable, and have strong clinical, assessment, and organizational skills, this can be the position for you. Responsibilities
- Provide one-on-one care for patients in their homes
- Conduct clinical assessments and develop patient care plans
- Collaborate with the home care clinical team to ensure high-quality care
- Maintain organized patient records and documentation
- Adhere to company policies, safety standards, and regulatory requirements
- Registered Nurse (RN)
- Satisfactory background checks and references
- CPR certification
- Driver's license and insurable driving record for company auto use
- Some nursing experience preferred; home care experience not required
