- Manage and coordinate individualized care plans for LTAC patients.
- Assess patient conditions and monitor treatment progress.
- Collaborate with physicians, therapists, and social workers.
- Advocate for patients and facilitate communication among all parties.
- Ensure proper documentation and compliance with standards.
- Coordinate discharge planning and care transitions.
- Educate patients and families on care and health management.
- Optimize use of healthcare resources.
- Participate in quality improvement and staff education efforts.
- Active Registered Nurse (RN) licensed with in the state of Indiana
- 2 years of recent and related experience, as required by facility
- Must be able to work independently, yet together as part of a team
- Must be able to remain calm under pressure
- Proficient at taking vital signs and administering medication accordingly
- Active BLS, physical, TB test, and other medical compliance as required by facility
