- Providing skilled nursing visits in patients' homes under the direction of the RN Case Manager.
- Performing nursing procedures and treatments ordered by the physician and outlined in the plan of care.
- Monitoring patient conditions and reporting changes to the supervising RN.
- Reinforcing education provided by the RN to help patients and families manage their health with confidence.
- Administering medications and treatments within the LPN scope of practice.
- Accurately documenting all patient care and clinical observations.
- Collaborating with physicians, nurses, therapists, caregivers, and other members of the healthcare team.
- Helping patients maintain independence, dignity, and the highest possible quality of life.
- Graduate of an accredited practical nursing program and active LPN license in South Dakota
- Minimum of 1 year of LPN experience, ideally in home healthcare (preferred)
- CPR certification
- Knowledge of state and federal home health regulations
- Compassionate, dependable, and committed to providing exceptional patient care.
- Organized and able to manage their daily schedule independently.
- A strong communicator who enjoys building relationships with patients and families.
- A team player who values collaboration and continuous learning.
- Comfortable using electronic medical records and completing timely documentation.
- Reliable transportation, current driver's license, and current automobile insurance
