Asa Hospice Client Care Coordinator (CCC), you will:
- Facilitate new client enrollment withinitialandsubsequentscreening assessmentsand furthermaintainclient’s healthcare record
- Identifyresources to address client’s health, daily living, and safety needs
- Perform administrative tasks such as scheduling, logging, documenting all appointments and services
- Communicateto clients through telephone and computer
- Coordinate with RN and pharmacy medication set-ups and reminders
- Be a family liaison and coordinate with the family, designees, and providers
- Education:High School graduate or equivalent
- One year of related healthcare experience is required
- Experience in working in the senior field or hospice is preferred
- Must have a valid driver’s license
