Asa Hospice Client Care Coordinator (CCC), you will:

• Facilitate new client enrollment withinitialandsubsequentscreening assessmentsand furthermaintainclient’s healthcare record
• Identifyresources to address client’s health, daily living, and safety needs 
• Perform administrative tasks such as scheduling, logging, documenting all appointments and services
• Communicateto clients through telephone and computer
• Coordinate with RN and pharmacy medication set-ups and reminders
• Be a family liaison and coordinate with the family, designees, and providers
  
  

To qualifyto be a Hospice Client Care Coordinator (CCC) with us, you will need:

• Education:High School graduate or equivalent
• One year of related healthcare experience is required
• Experience in working in the senior field or hospice is preferred 
• Must have a valid driver’s license
