What You'll Do
As a Home Health PT, you will deliver personalized therapy while helping patients safely regain function and independence:
Evaluate mobility, strength, balance, pain, and functional limitations in the home environment
Develop and update individualized therapy plans aligned with physician orders and patient goals
Provide therapeutic exercise, hands-on interventions, gait training, and mobility instruction
Recommend and train patients on appropriate assistive devices and safety strategies
Educate patients and caregivers on exercises, fall prevention, and safe techniques
Track progress, adjust interventions, and maintain timely, accurate documentation
Collaborate with nursing, occupational therapy, speech therapy, and other care team members
What You Bring
Graduate of an accredited physical therapy program with an active Arizona PT license
Home Health Experience Preferred
Minimum of 1 year of physical therapy experience
Current CPR certification
Valid driver's license, auto insurance, and reliable transportation
Strong clinical judgment, communication, organization, and documentation skills
