Position OverviewWe are seeking an experienced Registered Nurse (RN) Case Manager with a strong clinical background in patient assessment and care planning to join our team.
This role is ideal for a nurse looking to step into a more independent position while still utilizing hands-on clinical skills. As an RN Case Manager, you will conduct initial patient assessments, identify care needs, and develop individualized care plans. You will play a key role in coordinating care, monitoring patient progress, and ensuring effective communication between patients, families, and healthcare providers.
Key Responsibilities- Perform comprehensive initial assessments in patients’ homes
- Develop and implement personalized care plans based on patient needs
- Provide hands-on nursing care, including wound care, blood draws, and clinical interventions
- Coordinate care with interdisciplinary teams to ensure optimal patient outcomes
- Monitor patient progress and adjust care plans as needed
- Educate patients and families on treatment plans and ongoing care
- Ensure compliance with healthcare regulations and documentation standards
- Full-time: Monday–Friday, 8:00 AM – 5:00 PM
- Occasional on-call rotation (approximately once per month, based on needs)
- Travel required within Roanoke, Salem, Rocky Mount, and Moneta areas
- Current Registered Nurse (RN) license (Virginia or compact state)
- Strong clinical experience in:
- Wound care
- Blood draws
- Hands-on patient care
- Patient assessments and care planning
- Ability to work independently and effectively manage your own schedule
- Strong organizational, communication, and problem-solving skills
- Experience in home health, hospice, or related settings preferred
- BSN preferred
