PRN Hospice Nurse Practitioner
We are seeking an experienced Nurse Practitioner to join our growing hospice care program. This is an opportunity to make a meaningful impact by supporting patients with complex, chronic, or serious illnesses in a collaborative, patient-centered environment.
Position Details:
- This position is PRN with the opportunity to grow into part of full-time if desired.
- PRN visit rates mileage
- 401k with 4% match
Requirements:
- Minimum 1 year of experience managing complex patients, including independent prescribing experience
- Experience completing Face-to-Face (F2F) encounters for hospice patients
- Active Nurse Practitioner license in Wisconsin
- Strong clinical assessment, documentation, and care planning skills
- Experience with eClinicalWorks (eCW) EMR preferred but not required
- Excellent communication and teamwork abilities
Responsibilities:
- Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms
- Complete F2F encounters for hospice patients as needed
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Prescribe medications and coordinate care for patients with advanced or chronic illnesses
- Accurately document all encounters in the EMR
- Communicate effectively with patients, families, and other healthcare professionals
