Provide occupational therapy services in facility-based settings, including acute care, SNFs, ALFs, and rehab centers

Evaluate patients and develop individualized care plans based on medical needs and therapy goals

Use therapeutic interventions to improve functional abilities, mobility, and independence

Recommend adaptive equipment and strategies for improved safety and quality of life

Document patient progress and collaborate with facility staff to adjust plans as needed

Participate in interdisciplinary care planning and support patient-centered goals
What You Need:
-
Graduate of an accredited Occupational Therapy program
-
Active OT license in the state of WI
-
Minimum of one year of experience in a facility-based setting (acute care, SNF, ALF, or rehab)

CPR certification (American Heart Association or American Red Cross)

Clinical competence, solid judgment, and strong communication skills

Professionalism, flexibility, and compassion
