As a Home Health Licensed Practical Nurse (LPN), here’s a big-picture view of what you’ll do:
- Provide home-based care under the supervision of a RN to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility
- Work with a team of physicians, RN’s, and CNAs to deliver quality patient care
- Examine patients, complete assessments, document progress and report changes to their physician
- Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
- Coach family members on the patient’s plan of care, medication and home safety
- Collaborate with interdisciplinary team to coordinate patient care and ensure goals are met and plan for discharge from services
A few must-haves for Home Health LPN's:
- Graduate of a practical (vocational) nursing program for LPN and has a current GA LPN license
- Minimum of 1 year (LPN) experience in home healthcare
- HCHB charting experience highly preferred
- CPR certification, negative ppd screening or chest x-ray
- Drivers license, reliable transportation & auto insurance
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing specialty, strong communication skills and compassion
- Able to pass federal and state required criminal / abuse background checks and drug screen
