Job Summary
Makes in-home nursing visits to the elderly and disabled providing state authorized nursing services.
Monitors provision of personal care services to clients through on-site visits, Monitors quality of service to clients through on-site visits.

Essential Job Functions
1. Under the direction of an RN, makes supervisory on-site visits to evaluate the adequacy of authorized services to meet the needs and conditions of personal care clients and assess the worker’s ability to carry out authorized services.
2. Provides professional LPN services such as:
a. Skin monitoring and care
b. Set up med-boxes and administer prescribed medications prn
c. Nail care for the diabetic client or client with circulatory or neurological deficiency
d. Complete General Health Evaluations
e. Give injections
f. Take vitals, complete client assessment and call PCP as needed
g. Catheter changes
h. Evaluate Advanced Personal Care
3. Fills out and completes appropriate paperwork including Nurse Visit Reports, Physician’s orders, Nurse Visit Log, billing sheets, time sheets and mileage forms.
4. Keeps client files updated including med lists, addresses and phone numbers.
5. Assists the Personal Care Aides to solve problems/obstacles that interfere with adequate personal care.
6. On-call rotation answering office telephone after hours.
Job Type: Part-time, 10-15 hours per week. Make your own hours
