Starting Pay Range: $55,000

Position Type: FTE (37.5 hours/week)


PURPOSE: 

Provide strengths-based, person-centered therapy to older adults within the community and over telehealth as needed. Treatment to include diagnostic assessments; individual, group, and family therapy; and supportive services for a diverse caseload of older adults experiencing a variety of behavioral health issues.

 

FUNCTIONS:

  1. Provide client-centered, trauma-informed, mobile mental health treatment to a caseload that will not exceed 25 individuals.
  2. Enter intake and therapy appointments into EMR at least one week in advance.
  3. Meet minimum performance standards, including a minimum billing expectation of 18 - 20 hours per week. All progress notes need to be entered by the end of the business day on Fridays in order to have met this minimum expectation.
  4. With the individual’s consent, make a concerted effort to engage identified support system including family and significant others, as it pertains to identified treatment goals.
  5. Provide an initial and ongoing assessment of presenting issues, diagnostic impressions, and recommended course of treatment.  
  6. Create treatment plan and routinely monitor progress in collaboration with the individual.
  7. Provide linkage identified resources as needed. This may include coordination with Care Coordinator to facilitate referrals for expanded mental health programming including IOP, D&A, BCM, Psychiatry or community services.
  8. Coordinate care with other involved professionals (including Senior Community Centers and psychiatric, medical, or social services providers, etc.) to ensure holistic, integrated care.
  9. Actively participate in staff meetings and provide consultation/support to team members.
  10. Create a safety plan and provide crisis intervention as needed. This will include follow up and coordination of care when individuals go to the CRC or another acute level of care.  
  11. Maintain required records and documentation, including intake assessments, progress notes, treatment plans, and discharge summaries in compliance with Agency, State, and County regulations.
  12. Assist with community outreach, education and prevention programs as needed.
  13. Complete all mandatory trainings (in first 90 days and then as needed) to maintain appropriate credentialing; obtains EBP training as requested by supervisor.
  14. Function in compliance with agency policies/procedures and federal, state and local laws.
  15. Attend regular supervision with Older Adult Outpatient Supervisor following supervision policy. Come to supervision prepared to discuss current caseload, weekly productivity and any relevant clinical or administrative questions or concerns.
  16. Actively participate in appropriate EBP consultation group(s), each held for one hour/month.
  17. Embrace Sanctuary Meeting and behave in a manner that promotes safety, equity, anti-ageist beliefs, and trauma-informed practices.        
  18. Attend internal and external meetings as requested by supervisor.
  19. Other duties as assigned.

 

QUALIFICATIONS:


      1. Master’s or Doctoral degree in Human service field.  

  1. Must be clinically licensed or licensed-eligible in the Commonwealth of PA. If license eligible, will obtain clinical license in no more than 2 years.
  2. At least 1 year of experience providing behavioral health therapy to older adults including a demonstrated knowledge of the specific nuances of treating this population.   

 

ADDITIONAL REQUIREMENTS: 

  1. Must have Valid Childline, PA Criminal History and FBI Clearances prior to seeing clients. 
  2. Previous training in EBPs or a willingness to obtain training. 
  1. Reliable means of transportation and valid driver’s license from state of residence with a minimum of one (1) year driving experience and a driving record free of at-fault accidents or moving violations for the past year.
  2. Ability to work collaboratively within a team-oriented environment.
