Our team partners closely with physicians, hospitals, and community providers to ensure safe transition of care, prevent hospital readmissions, and improved patient outcomes.
Job Summary
We are seeking a driven and relationship-focused business development manager to expand our patient census through strategic outreach and referral development. This role is ideal for someone who understands the healthcare landscape and can build lasting relationships and partnerships with referral sources such as physicians, hospitals, skilled nursing facilities, and case managers.
Duties
- Develop and maintain strong referral relationships with physicians, specialty clinics, hospitals, discharge planners, skilled nursing facilities (SNF), and assisted-living communities.
- Identify and pursue new referral opportunities to grow Medicare census
- Educate referral partners on home health eligibility, services, and admission criteria
- Coordinate with clinical intake team to ensure smooth patient transitions
- Represent the agency in the community, networking events, and professional meetings.
Qualifications
- Experience in home health for healthcare marketing /business development preferred
- Strong understanding of Medicare Home Health eligibility
- Established relationships with local referral sources (highly preferred)
- Self motivated, organized, and results-driven
