Role Description
This is a full-time hybrid role for a Field Nurse Case Manager, located in Shreveport, LA and surrounding areas. The Field Nurse Case Manager will be responsible for managing and coordinating patient care, conducting discharge planning, and overseeing case management activities. Tasks include assessing patient needs, developing care plans, and working closely with medical professionals, patients, employers, and attorneys to ensure optimal outcomes.
Qualifications
- Strong background in Nursing and Medicine, preferably with ER, trauma, ortho, or neuro experience.
- Proven problem solving skills
- Excellent communication and advocacy skills
- Ability to work independently and in a hybrid work environment
- Current RN license in Louisiana
- Bachelor's degree in Nursing
