Overview
We are seeking a dedicated and compassionate Resident Care Coordinator to join our team - all shifts available. In this role, you will be responsible for coordinating and managing the care of our residents, ensuring their well-being and safety. This role requires excellent organizational skills, strong communication abilities, and a passion for providing quality care.
Responsibilities:
- Coordinate and schedule resident appointments, ensuring all necessary services are provided
- Conduct assessments to determine resident needs and develop individualized care plans
- Monitor resident progress and make adjustments to care plans as needed
- Collaborate with healthcare professionals, caregivers, and family members to ensure coordinated care
- Provide guidance and support to caregivers, offering training and resources as needed
- Assist with medication management and administration
- Monitor vital signs and report any changes or concerns to healthcare providers
- Implement behavior management strategies for clients with challenging behaviors
- Maintain accurate and up-to-date client records
Qualifications:
- First aid certification required
- Experience in caregiving or senior care preferred
- Knowledge of assisted living facilities and home care services
- Strong organizational skills
- Excellent communication and interpersonal skills
- Ability to lift and assist residents as needed
- Valid driver's license with a clean driving record
