Integrity House operates 16 licensed treatment programs across New Jersey as well as community outreach and prevention programs, and serves over 3,000 individuals every year.

Integrity House is seeking a Care Coordinator to provide low-threshold services at the Integrity Wellness Center. Located in Newark, NJ, the Integrity Wellness Center provides a welcoming, stigma-free environment for people who use drugs, sex workers, and people living with HIV/AIDS and/or Hepatitis C to access health and wellness resources and support. The center offers risk reduction supplies and education, health services, peer support, care coordination, laundry and personal care services, and other resources at no cost.

Key Responsibilities

• Engage participants in a respectful, non-judgmental manner and build rapport to support ongoing engagement.
• Conduct screenings and assessments to identify participant needs across key domains including housing, healthcare, benefits, employment, and social supports.
• Develop and implement individualized service plans based on participant-identified needs and priorities.
• Use Motivational Interviewing and risk reduction strategies to support participant readiness and goal-setting.
• Contribute to the development and maintenance of community partnerships.
• Coordinate referrals and linkages to internal programs and external providers, including medical and social services.
• Support participants in navigating systems related to health insurance (e.g., Medicaid/Medicare) and public benefits (e.g., SSI/SSDI, SNAP, TANF).
• Facilitate access to services by scheduling appointments, conducting warm handoffs, and addressing logistical and systemic barriers.
• Conduct proactive follow-up to confirm service connection and address barriers to care.
• Distribute prevention supplies and educational materials in accordance with center protocols.
• Collaborate with team members to support service delivery.
• Maintain accurate, timely, and complete documentation of participant interactions, referrals, and outcomes.
• Participate in outreach activities as needed.
• Support data collection, reporting, and initiatives related to continuous quality improvement.
• Participate in supervision, team meetings, and training activities.
• Adhere to program policies, procedures, and service delivery standards.
• Perform other duties as assigned.
  
  

Qualifications

• High school diploma or GED.
• At least one (1) year of experience providing care coordination/case management services.
• Commitment to providing non-judgmental, person-centered care.
• Ability to build trust and engage individuals who may be ambivalent about health and/or social service systems.
• Experience working in community-based or low-threshold service settings.
• Experience working with individuals using substances, people living with HIV/AIDs, individuals living with Hepatitis C, sex workers, and individuals experiencing homelessness.
• Knowledge of trauma-informed care, Motivational Interviewing, and low-barrier engagement strategies.
• Ability to assess needs, prioritize tasks, and coordinate multiple services effectively.
• Ability to navigate systems related to healthcare, social services, and benefits.
• Sound judgment and ability to respond to complex or evolving participant needs.
• Ability to maintain accurate and timely documentation.
• Ability to work effectively as part of a team in a fast-paced, hands-on environment.
• Fluency in Spanish a plus.
  
  

Schedule:

The employee is expected to work Monday through Friday from 9 AM to 5 PM, however work during nontraditional hours may be needed in accordance with business and program needs.

Salary:

$19 – $22.50 per hour
