Registered Nurse Practitioner (Pediatric Home Care – One-on-One Care)
A Registered Nurse Practitioner (Pediatric) in a home care setting is responsible for providing individualized, one-on-one medical care to pediatric patients in their homes. The role focuses on performing routine health assessments, managing acute and chronic pediatric conditions, supporting developmental health, and collaborating closely with families and other healthcare professionals to ensure continuity of care in a safe home environment.
The Pediatric Nurse Practitioner provides family-centered care by educating caregivers, coordinating services, and promoting healthy lifestyle practices appropriate to the child’s age and developmental needs.
Responsibilities
- Conduct comprehensive pediatric assessments, including physical examinations and developmental observations in the home setting
- Examine pediatric patients and review their medical histories and care plans
- Order and interpret diagnostic tests (e.g., blood tests, X-rays, and other laboratory work) as clinically indicated
- Diagnose acute and chronic pediatric health conditions and illnesses
- Propose and manage treatment plans for chronic, developmental, and infectious diseases
- Provide primary care services tailored to pediatric patients in a one-on-one home care environment
- Perform initial and ongoing examinations, including monitoring growth, behavior, and physical condition
- Set up, operate, and monitor medical equipment used in the home (e.g., feeding pumps, oxygen equipment, suction machines)
- Administer medications and treatments as prescribed and monitor patient response
- Communicate diagnostic results clearly to patients’ parents or caregivers and explain recommended treatments
- Maintain accurate patient medical records, visit notes, and care schedules in compliance with healthcare standards
- Educate patients and family members on disease processes, infection prevention, medication administration, and overall pediatric health
- Provide guidance to caregivers on healthy lifestyle practices, nutrition, hygiene, and safety appropriate to the child’s age
- Monitor the patient’s overall health status and promptly address changes in condition
- Collaborate with physicians, therapists, home health staff, and other healthcare professionals to coordinate care
- Promote preventative healthcare practices, immunization compliance, and developmental health monitoring
- Participate in ongoing professional education and maintain competency in pediatric and home healthcare best practices
- Support family-centered care by addressing caregiver concerns and providing emotional support when needed
