Position Details:
- Location: Aiken, SC
- Schedule: Flexible – we work around your availability
- Type: Per Visit (Contract or W-2 options available)
- Compensation: Competitive per-visit rate (paid after documentation submission)
- Conduct initial nurse assessments for new clients
- Complete follow-up assessments at 30 days, 4 months, and as needed
Review care plans and ensure services meet client needs and Medicaid requirements - Communicate effectively with caregivers, clients, and office staff
- Document visits accurately in our CareSmart app (training provided)
Report any significant changes in client condition
- Active RN license in good standing with the South Carolina Board of Nursing
- Must have reliable transportation and own smartphone
- Ability to learn and use our mobile care documentation app
