Job Overview
We are seeking a compassionate, skilled, and self-motivated RegisteredNurse (RN) to join our Home Health team as aClinical Case Manager. In this role, you will provide high-quality nursing care to patients in their homes, following individualized care plans under physician direction. The Home Health RN plays a vital role in promoting patient recovery, independence, and overall well-being through personalized, evidence-based care.
Responsibilities
- Conduct comprehensive initial and ongoing patient assessments.
- Develop, implement, and continuously update individualized care plans in collaboration with physicians and interdisciplinary team members.
- Administer prescribed medications and treatments accurately and safely.
- Monitor and document patient responses, progress, and any changes in condition.
- Educate patients and caregivers on disease processes, self-care techniques, and preventive health practices.
- Coordinate services with other healthcare professionals, including physical therapists, social workers, and home health aides.
- Ensure compliance with all clinical, regulatory, and documentation standards (HIPAA, OSHA, CMS).
- Maintain timely, accurate, and complete records in the Electronic Medical Record (EMR) system.
- Participate in on-call rotations, team meetings, and interdisciplinary case conferences as required.
Skills
- Current and active RN license in the state of practice (required).
- Minimum 1 year of nursing experience, preferably in home health, hospice, or acute care.
- Current BLS certification (Basic Life Support).
- Valid driver’s license, reliable vehicle, and proof of auto insurance.
- Strong clinical judgment, communication, and time-management skills.
- Ability to work independently and manage multiple home visits daily.
- Proficiency in electronic charting systems (EMR); experience with OASIS and Medicare-certified home health documentation preferred.
- Ability to work collaboratively within a multidisciplinary team while demonstrating excellent patient service skills.
- Experience in outpatient clinic settings or post-acute care environments is highly desirable.
Physical Demands:
- Ability to travel to and provide care in patients’ homes and various environments.
- May involve lifting, transferring, or repositioning patients.
- Frequent standing, walking, bending, and reaching required.
