About the Opportunity
">In this role, you will join our Outpatient Rehab team at INTEGRIS Health Ponca City in Oklahoma, providing exceptional care to those who have entrusted us with their healthcare needs.
">We are seeking a highly skilled Physical Therapist to work 8am-5pm, Full-time. The ideal candidate will be responsible for assuming care, managing and coordinating patient treatment as directed by the referring physician.
">The selected candidate will also be required to adhere to National Patient Safety Goals appropriate based on the level of patient contact.

">Responsibilities
">- Complete initial evaluation and treatment of physical therapy patients
- Create a set up plan of care and goal for physical therapy after initial evaluation
- Document physical therapy patients in accordance with the guidelines of the Rehab Department policies
- Supervise PTA and PT aide
Requirements
">- Bachelor's degree from program accredited by the American Physical Therapy Association preferred
- Current Oklahoma Physical Therapy license OR be eligible for license in the state of Oklahoma AND able to practice under the Oklahoma Physical Therapy Practice Act. Must have completed licensure by 6 months after initial temporary license is issued.
- CPR Certification
- Experience in Physical Therapy preferred
- Must be able to communicate effectively in English
- Valid Driver's License from the state of residence, driving record which is acceptable to our insurance carrier and auto insurance verification
