The Medical Social Worker provides psychosocial assessments, crisis intervention services and counseling to patients. This position coordinates healthcare services throughout the continuum of care on behalf of the patient and assists in discharge planning. This position requires population specific competencies. REQUIRED QUALIFICATIONS
EDUCATION:
- Master's in Social Work (MSW) from an accredited school
Must be able to communicate effectively in English (verbal/written).Hospice and Home Health: This job requires the incumbents to operate an INTEGRIS-owned vehicle OR personal vehicle (non INTEGRIS-owned) and therefore must have a current Oklahoma State Drivers License as well as a driving record which is acceptable to our insurance carrier.

- Professional experience
- Experience in a medical setting
- NZTI only: Prefer related experience in a transplant institute
