Responsibilities
- Assess patients’ physical conditions and develop tailored care plans to meet individual needs
- Implement therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to enhance mobility and reduce pain
- Conduct discharge planning to ensure smooth transitions from hospital or outpatient settings to home care
- Monitor patient progress regularly, adjusting treatment plans as necessary to optimize outcomes
- Document all clinical activities accurately in electronic health records (EHR) systems following clinical documentation standards
- Collaborate with interdisciplinary teams including physicians, nurses, and caregivers to coordinate comprehensive care
- Educate patients and their families on physiotherapy techniques, safety measures, and home exercise programs
Qualifications
- Valid license or certification to practice as a physical therapist in the relevant jurisdiction
- Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings
- Strong knowledge of physiology, anatomy, neurology, orthopedics, and geriatrics to inform effective treatment strategies
- Familiarity with medical terminology and clinical documentation standards for accurate record keeping
- Experience working with diverse patient populations including geriatric, pediatric, neurological, and orthopedic cases
- Proficiency in electronic health records (EHR) and electronic medical records (EMR) systems for seamless documentation and communication
- Ability to perform discharge planning and coordinate care across multiple healthcare environments
- Excellent patient care skills with an energetic approach to motivating patients toward recovery
