Dual Care Coordinator

Full-Time | Hybrid (Remote & Field-Based)

Position Overview

At Integrative Health Care Services of Indiana, our mission is to improve the quality of life for seniors and individuals with disabilities by providing compassionate, coordinated, and person-centered care.

The Dual Care Coordinator serves as an advocate, educator, and trusted partner for clients by coordinating services, removing barriers to care, and promoting positive health outcomes. This position works collaboratively with clients, families, healthcare providers, and community partners to ensure individuals receive the resources and support needed to safely and successfully remain in their homes and communities.

Success in this role requires compassion, accountability, excellent communication, strong critical thinking skills, and the ability to build trusting relationships while managing a diverse caseload in a fast-paced environment.

Key Responsibilities

The Dual Care Coordinator is responsible for:

  • Developing, implementing, and monitoring individualized care plans based on each client's unique needs and goals.
  • Conducting monthly face-to-face visits with assigned clients and completing timely, accurate documentation.
  • Assessing client needs, identifying barriers to care, and coordinating appropriate interventions.
  • Collaborating with physicians, healthcare providers, caregivers, community agencies, and internal team members to ensure continuity of care.
  • Scheduling and coordinating appointments, referrals, and services as needed.
  • Educating clients and families regarding available resources, healthcare services, and care plan goals.
  • Monitoring client progress and adjusting care plans as appropriate.
  • Maintaining accurate and complete client documentation in accordance with company policies and regulatory requirements.
  • Advocating for clients while promoting independence, dignity, and informed decision-making.
  • Maintaining confidentiality and complying with HIPAA and all applicable federal and state regulations.
  • Performing additional duties as assigned to support departmental and organizational objectives.

Minimum Qualifications

  • Valid driver's license with reliable transportation and current automobile insurance required.
  • Ability to travel regularly throughout an assigned service region within Indiana.
  • Experience working with seniors, individuals with disabilities, or other vulnerable populations preferred.
  • Experience in care coordination, case management, healthcare, social services, or a related field preferred.
  • Bilingual in English and Spanish preferred to better serve our diverse client base.
  • Strong written and verbal communication skills.
  • Excellent organizational and time management abilities.
  • Comfortable using computers, electronic documentation systems, Microsoft Office, and other technology required to perform job duties.
  • Ability to work independently while effectively managing priorities in a hybrid work environment.
  • REQUIRED - Must be located in Fort Wayne, Indiana or surrounding area. This position requires client facing visits across the Fort Wayne, Region.

Work Environment

This is a full-time hybrid position consisting of remote work and field-based responsibilities. Dual Care Coordinators are expected to travel throughout their assigned region in Indiana to conduct client visits, coordinate services, and maintain community partnerships.
