The RN Case Manager is responsible for coordinating comprehensive care and services for patients across the continuum of care to ensure optimal health outcomes, appropriate resource utilization, and patient satisfaction. This role involves clinical assessment, care planning, advocacy, and collaboration with interdisciplinary teams to support patients through transitions in care.
- Conduct comprehensive assessments of patients’ medical, psychosocial, and functional needs.
- Develop individualized care plans in collaboration with patients, families, and the multidisciplinary healthcare team.
- Coordinate medical services, specialist referrals, discharge planning, and follow-up care.
- Monitor patient progress and modify care plans as necessary to ensure optimal outcomes.
- Facilitate communication among healthcare providers to prevent gaps or duplication in care.
- Educate patients and families about conditions, treatment plans, and available community resources.
- Ensure compliance with regulatory and organizational standards, including HIPAA and applicable laws.
- Document all case management activities accurately and timely in the electronic health record.
- Participate in quality improvement initiatives, audits, and case reviews.
- Serve as a clinical resource and advocate for evidence-based, patient-centered care.
EDUCATION/EXPERIENCE/LICENSURE REQUIREMENTS
Graduate of an accredited School of Nursing. Associate’s Degree (AAS) required and Bachelor’s (BSN) preferred
Minimum 2 years experience in Case Management preferred.
Current CPR certification; CCHP or CCHP-A; NCCHC accreditation a plus, Current CPR Certification, Current unrestricted California RN license required
