Contract: 13 weeks of contract
Shift: 5 Days/Week | 8:00 AM–4:30 PM
Pay Rate:
- Local: $45/hr on W2
- Travel: $2,100.05/Weekly Gross | Stipends: $1,140.05/Weekly included
Job Summary
Seeking an experienced Hospital Care Management professional to coordinate patient care, discharge planning, resource utilization, and multidisciplinary services. The role focuses on assessing patient needs, developing individualized care plans, coordinating services, and supporting efficient transitions of care.
Key Duties
- Coordinate care and services for patients meeting Care Management criteria.
- Conduct comprehensive assessments of patients’ medical, functional, and social needs.
- Develop individualized care plans and establish goals with patients, families, and caregivers.
- Coordinate and support discharge planning and transitions of care.
- Assess prior level of functioning, community resources, and available support systems.
- Ensure appropriate services, care settings, continued care, education, and community resources.
- Collaborate with physicians and multidisciplinary healthcare teams.
- Identify appropriate admission and continued-stay issues.
- Promote effective and efficient utilization of healthcare resources.
- Identify strategies to reduce length of stay and improve appropriate service utilization.
Requirements
- Minimum 1 year of clinical experience – Required
- Hospital Care Management experience – Required
- LCSW license in Montana – Required
- 2 years of hospital-based case management experience – Preferred
- ACM or CCM certification – Preferred, not required
