Duration: Direct Hire
Schedule: 8:00 AM–4:00 PM or 9:00 AM–5:00 PM | 37.5 Hours/Week
Pay Range: $95,713/Year
Work Arrangement: On-site
Job Summary:
The LCSW will provide trauma-focused psychosocial assessments, crisis intervention, patient and family support, behavioral health referrals, and discharge planning for injured patients throughout hospitalization and after discharge.
Key Responsibilities:
- Assess trauma patients and identify psychosocial and behavioral health needs.
- Provide crisis intervention, PTSD screening, and patient/family support.
- Coordinate discharge planning, community resources, and behavioral health services.
- Collaborate with trauma surgeons, nursing, and multidisciplinary care teams.
- Document assessments, interventions, and follow-up in the electronic medical record.
- Participate in trauma rounds, case conferences, education, and performance improvement activities.
Required Qualifications:
- Master’s degree in Social Work or a related field.
- Active New York State Licensed Clinical Social Worker (LCSW) license and registration, verified through NYSED.
- Experience with trauma-informed care, crisis intervention, psychosocial assessments, and complex discharge planning.
- Knowledge of behavioral health, PTSD, substance use, and community resources.
- Strong communication and multidisciplinary collaboration skills.
Preferred Qualifications:
- Trauma certification.
- Experience supporting trauma patients, including PTSD screening, family support, and post-discharge follow-up.
Salary : $94,500 - $95,713
