Job Title: RN Care Transition Manager

Location: Providence, RI

Duration: 3 months (possible extension)

Shift: 08:00 AM – 04:30 PM, 40 Hours per week


Compensation:

Local Pay - $70/hr


Travel Pay:

Overall Weekly Pay: $3,032.5

W2 Hourly Pay Rate: $34.9

Weekly Stipend: $1,636.5


Requirements:

  • BSN or equivalent required
  • A state professional registered nurse license or multi-state compact license is required.
  • Minimum 5 years of acute care or case management experience
  • Strong knowledge of utilization review, discharge planning, and payer regulations


Job Overview:

We are seeking an experienced RN Care Transition Manager to support a Transitions of Care team through telephonic patient follow-ups post-discharge from acute care settings. This role focuses on ensuring continuity of care, appropriate resource utilization, and reducing readmissions while meeting quality and timeliness benchmarks.


Key Responsibilities:

  • Conduct post-discharge and ED follow-up calls within required timelines (including 48-hour TCM calls)
  • Coordinate with physicians, care coordinators, and social workers to ensure smooth care transitions
  • Support discharge planning and facilitate referrals (SNF, home health, outpatient services, infusion therapy)
  • Identify barriers to care and collaborate on solutions
  • Maintain accurate documentation in Epic and Cerner
  • Support initiatives to improve patient flow and reduce readmissions
  • Ensure full compliance with HIPAA regulations
