About the Role:The RN Case Manager plays a critical role in coordinating and managing patient care to ensure optimal health outcomes and efficient use of healthcare resources. This position involves comprehensive assessment, planning, implementation, and evaluation of patient care plans in collaboration with multidisciplinary teams. The RN Case Manager serves as a liaison between patients, families, healthcare providers, and insurance companies to facilitate seamless care transitions and advocate for patient needs. By monitoring patient progress and outcomes, the RN Case Manager identifies barriers to care and implements strategies to overcome them, promoting patient safety and satisfaction.
