What are the responsibilities and job description for the Beneficiary Care Coordinator position at INTEGRATED COMMUNITY SERVICE?
ICS currently has a license for a Home Health Agency (HHA) in the District of Columbia and is currently accepting applications for a Beneficiary Care Coordinator
You will be responsible for:
- Establish and develop schedules and monitor the Personal Care Assistants (PCAs) work schedules.
- Develop and coordinate the field monitoring plans for PCAs and Beneficiaries as approved by Supervisor.
- Perform announced and unannounced visits/phone calls to ensure HHAs are reporting for their scheduled shifts and are properly performing their assigned duties.
- Approve and schedule time off for HHAs and ensure vacant positions are filled to meet staffing needs.
- Create and maintain weekly schedules in the company software.
- Review all timecards in software for PCAs working on assigned caseloads on a weekly basis.
- Be on-call via telephone 24 hours to respond to emergency scheduling needs to ensure staffing requirements for each Beneficiary are met to comply with internal and external policies.
- Establish and maintain an on-going professional relationship with beneficiaries and beneficiaries circle of support.
- Complete weekly staffing report and submit to supervisor as required.
- Collect, and review timesheets to match schedules before submitting to the billing department.
- Monitor PCA’s hours worked before assigning he/she a case to ensure that PCA does not work more than 16 hours per day.
- Complete weekly staffing reports, verifying employees’ work hours, requested time off as well as staffing needs. This report will be summited to the Supervisor per the established schedule.
- Plan, organize and implement a sustainable staffing strategy.
- Investigate the company’s staffing needs and take actions to staff accordingly.
- Provide after hours and weekend coverage as appropriate.
- Review the HHA beneficiary profile prior to being able to communicate all relevant medical information for new and existing beneficiaries.
- Identify appropriate cover personnel for last minute call outs then reach out to the beneficiary or family if cover personnel is needed as appropriate.
- Address PCA and beneficiary issues and escalate as appropriate.
- Perform monthly audits of beneficiary charts to ensure timesheets and suspension forms are up to date and filed appropriately.
- Maintain call log of customer service calls and PCA on duty tracking.
- Track and report hospital, rehab and incidents admissions and discharges.
- Document and report all missed HHA visits and beneficiary suspension of services with the appropriate reason.
- Ensure original timesheets are received and accurate, signed by PCA and beneficiary and processed in a timely manner.
Work Hours/Shift:
9AM-5:30PM Full Time
Requirements and Qualifications
High School Diploma
Computer knowledge required – Microsoft Office Suite
One year of medical office experience preferably with staffing
Outstanding organizational and time-management skills
Interpersonal skills
Excellent communication, interpersonal and leadership skills
Critical thinker with a problem-solving mind
Customer Service
Passion for providing service
Preferred Qualifications
Two (2) years of experience as a Staffing Coordinator or similar role.
Knowledge of staffing trends and best practices
Some college a plus
2nd Language Spanish a plus
Summary
The BCC provides support to all HHA departments and works collaboratively with all department teams. Maintains a positive working relationship with staff at all levels to ensure adequate care coordination, in accordance with current applicable federal, state and local standards, guidelines and regulations as well as ICS established policies and procedures.
