Job Summary

The Physical Therapist (PT) is responsible for evaluating, planning, and implementing individualized physical therapy treatment programs for patients in the home setting. The PT assesses functional deficits related to mobility, strength, balance, endurance, and pain, and develops goal-directed Plans of Care to maximize patient independence and safety. The PT collaborates closely with the interdisciplinary care team, supervises Physical Therapy Assistants (PTAs), and ensures all services are delivered in accordance with Medicare, Medicaid, and agency standards.

Duties· Conduct comprehensive physical therapy evaluations and assessments in the home environment

· Develop and implement patient-centered, individualized Plans of Care (POCs) based on patient goals and physician orders

· Provide skilled therapeutic interventions including therapeutic exercise, gait training, balance and fall prevention, neuromuscular re-education, manual therapy, functional mobility training, and pain management

· Complete OASIS SOC, ROC, Recertification, and Discharge assessments according to Medicare-required timelines

· Educate patients and caregivers on home exercise programs, safety strategies, fall prevention, activity modifications, and disease management

· Complete timely and accurate clinical documentation (assessments and visit notes) in compliance with Medicare, Medicaid, and payer requirements using Axxess EMR

· Participate in care conferences, case management, and discharge planning

· Maintain compliance with all federal, state, and agency policies and regulations, including:

· Medicare Conditions of Participation

· DC licensure and regulatory requirements

· ICS policies and documentation standards

· Communicate changes in patient condition to the interdisciplinary team and primary care physician

· Monitor and document patient progress, adjusting treatment plans as clinically indicated

· Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the care team to ensure continuity of care

· Supervise Physical Therapy Assistants (PTAs) in accordance with DC practice requirements

· Participate in staff in-services and mandatory trainings including: HIPAA Compliance, Infection Control, OASIS/documentation reviews, Regulatory changes, Fall Prevention, Fraud/Waste & Abuse, and Abuse/Neglect & Exploitation

· Participate in and document care conferences, case management, discharge planning, and interdisciplinary care coordination with all assigned clinicians, PCP, case managers, family/caregivers, aides, and other involved providers

Experience

  • · Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy – DPT required)

· Minimum one (1) year of clinical experience preferred; strong new graduates considered

· Home health experience preferred but not required

· Strong clinical reasoning, documentation, and communication skills

· Ability to work independently and manage a flexible home health caseload

· Familiarity with OASIS documentation (training available)

· Experience in acute care, outpatient, or community-based rehabilitation is a plus

Pay: $1.00 - $2.00 per year
