What are the responsibilities and job description for the Registered Nurse Case Manager position at INTEGRATED CARE COMMUNITIES?
RN Case manager position for Developmentally Disabled adults in Moreno Valley 3 ICF DDH facility's and 1 DDN Facility.

- Monitor, assessor and Motivator.
- RN will be in charge of 3 ICF DDH Homes and 1 ICF DDN Homes. Each home can hold up to a total of 6 clients.
- Responsible for ensuring compliance to all federal and state regulations and requirements.
- Prepare nursing care plans for all clients
- Prepare annual and semi-annual reviews for the interdisciplinary team meetings
- Participate as a member of the ID team
- Participate in the development of each Individual Service Plan (ISP).
- Coordinate physician, specialists, dentist, pharmacy, lab and other health related services.
- Train DCCI and day program staff in the health care needs of clients as needed. Schedule all in-services as required by state and federal regulations.
- Monitor medication cabinets to insure compliance with state and federal regulations.
- Monitor medication administration record every 2 weeks; monitor consumer flow sheets as needed.
- Full Time Monday-Friday Position, On call for any health issues related to the client.
Pay: $85,000.00 - $90,000.00 per year
