This position requires the employee to reside in the state of Texas.
Job Summary: This position involves providing intensive case management and facilitating Wraparound services in accordance with the Wraparound Model developed by the National Wraparound Initiative Center.
Key Responsibilities Include
- Assisting youth and their families in identifying and accessing appropriate mental health, medical, social, educational, financial, and other services and supports.
- Assessing youth needs and strengths to authorize services and manage ongoing service authorizations.
- Coordinating service delivery with a multi-disciplinary team to provide an array of services to children and youth enrolled in the Youth Empowerment Services Waiver program.
- Overseeing the development of the Wraparound Plan and Safety Plan, including crisis prevention and management as needed.
- Monitoring service delivery and ensuring the development and coordination of the wraparound team for each youth on the caseload.

Education
- Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred.
Required:
- Current and valid driver's license and a clean driving record.
Preferred:
- Bilingual in Spanish.
Workdays: Monday - Friday.
Workhours: 8:00AM - 8:00PM.
Salary: $24.18 - $25.39 an hour based on education.

- Second Shift: $3.25/hour.
- Third Shift: $7.25/hour.
- Weekend Shift: $8.00/hour.
