Job Summary: The ACT Care Coordinator provides psychosocial rehabilitative skills training to individuals diagnosed with severe mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions, utilizing a recovery-based and trauma-informed perspective. Care Coordinators operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill-set by forging therapeutic relationships with diverse community members. Interventions and services are typically provided in the community and require the use of a personal automobile and agency assigned cell phone.

Care Coordinators work to define and facilitate their desired life outcomes. Care Coordinators participate in diagnostic evaluations and provide assessment of strengths, preferences, functional impairments, and symptoms of psychiatric and/or co-occurring disorder(s) at regular intervals to ensure optimal and personalized services. Frequent service coordination with external and internal providers is necessary to support individuals in achieving independent functioning. Care Coordinators may provide all or some of the following: deliver therapeutic interventions fostering skills necessary to manage symptoms, obtain/maintain employment or housing, or to obtain services such as school, medical care, nutritional assistance, financial assistance, transportation, legal assistance, and resources fulfilling any basic need. Care Coordinators may also facilitate psychosocial rehabilitative skills training groups and/or utilize other evidence-based interventions or therapeutic modalities.

Qualified Mental Health Professionals (QMHP) embrace trauma-sensitive and informed practices, provide Suicide Safe care, and foster a supportive and welcoming environment. QMHPs operate in a dynamic environment, develop extensive knowledge of community resources, collaborate with interdisciplinary teams, and cultivate a clinical skill set by forging therapeutic relationships with community members and/or stakeholders. Interventions and services can be both office- and community-based and may require the use of a personal automobile. Individuals served may be diagnosed with mental illness, co-occurring substance use disorders, chronic health conditions, and/or IDD.

Staff members in this position are working under the supervision of a clinically licensed professional who provides ongoing evaluation and oversight of all assessments, diagnoses, and treatment. At this level, clinical assignments may be of moderate complexity and are handled with considerable independence, and complicated cases receive closer clinical review and supervision.

Services provided may include individual, family, and/or group skills training using evidence-based interventions, crisis assessment, and case management.

Work is performed under general supervision with moderate latitude for the use of initiative and independent judgment.

Regular, predictable attendance is considered to be an essential requirement for this position. Other duties may be assigned. May require a flexible work schedule.

Education

  • Graduated from an accredited college or university with a Bachelor's degree with a major specialization in Human/Behavioral/Social Services or related field. Master's degree preferred.

License

Required:

  • Current and valid driver's license and a clean driving record

Experience

Preferred:

  • Bilingual in Spanish

Hours: Full-Time.

Workdays: Monday - Friday.

Workhours: 8:00 AM - 5:00 PM.

Salary: $24.18- $25.39 an hour based on education, plus an additional 10% crisis stipend.
