Job Overview:
We are seeking a dedicated Registered Nurse (RN) to join a growing home health team serving patients. This is a field-based role ideal for RNs who thrive on independence, critical thinking, and one‑on‑one patient care in the home setting.
Key Responsibilities:
- Manage and coordinate patient care plans
- Perform in-home patient assessments and visits
- Collaborate with interdisciplinary care teams
- Monitor patient progress and adjust care plans
- Ensure documentation and compliance standards
- Educate patients and families on care plans
- Deliver compassionate, patient-centered care
Requirements:
- Active RN License (Virginia or compact)
- BLS (AHA) within 90 days
- Associate Degree in Nursing
- At least 2 years RN experience
- At least 1 year Home Health experience (REQUIRED)
- Reliable transportation required for patient visits
Pay: $75,000.00 - $90,000.00 per year
