Responsible for supporting IOA Lifeways Program and Program Director by performing administrative duties related to planning, coordinating, monitoring and implementing PACE operations. Acts as liaison for new participants and staff.
Specific Responsibilities
Range: $26 - $32/hourly

Specific Responsibilities
- Intake and Enrollment
- Works closely with On Lok Enrollment Specialist and Intake RN on monitoring each candidate’s process and updates Interdisciplinary Team (IDT) on status.
- Supports Program Director in scheduling and monitoring all Intakes and Enrollments for the program.
- Updates internal intake spreadsheet and calendar and informs teams of schedules for Friendly Visits, Home visits. Provides information from MD and SW visits and assists with assignment to team.
- With direction from Program Director, acts as liaison for new participants and families to ensure needs are met.
- Works to engage new candidates/participants and their families by introducing them to staff and other participants.
- Schedules 30-day follow-up for team and Program Director to meet with newly enrolled participants for satisfaction of program and as needed, care planning and status updates.
- Reminds leadership team of performance and competency due dates.
- Ensure compliance with three tracking spreadsheets (Phone Tree, Employee Qualifications, Required Training)
- Support completion of New Hire Orientation, assist with scheduling as needed.
- Schedules, coordinates and documents staff training including CPR, BLS, Required Trainings, and other trainings and qualifications as needed.
- Assists Director and Quality Improvement Associate in audit preparation and ongoing monitoring tasks as well as coverage as needed for tracking.
- Monitors attendance tracking.
- Supports program operations to ensure daily participant needs are met as requested by such as assisting with transportation requests, follow up with billing issues, assisting with hospital discharges and/or form completion and completing administrative tasks as requested by Leadership Team and as approved by Program Director.
- Completes RCFE Care Plan form for review by participants RCFE.
- All other related jobs as assigned.
- Bachelor’s degree in health or social science related field or equivalent experience.
- One year experience working with older adults and/or adults with disabilities.
- Demonstrate strong problem solving skills, resourcefulness, show ability to take initiative at all times.
- Strong leadership and communication skills.
- Have a good understanding and knowledge of the requirements of the PACE Lifeways program as well as the structure and functions of the department
- Be able to work independently and to be organized in their work to ensure that tasks are completed in a timely manner
- Ability to work within the interdisciplinary team to support efforts of PACE.
- Ability to structure and conduct audits and chart reviews for both staff and participants.
- Strong knowledge in information systems, Microsoft Word, Microsoft Excel, PowerPoint programs, electronic and voicemail.
- Cultural competency and the interest and ability to work within a multilingual, multicultural environment.
- Ability to act as an advocate for frail elders.
- Bilingual Cantonese strongly preferred.
- Ability to lift 25 lbs.
- Ability to reach, bend and walk
