River Valleys Continuum of Care (MN-502) is the planning and leadership entity supporting twenty counties of southern Minnesota in their efforts to prevent and end homelessness together. As a federally-recognized Continuum of Care (CoC) region, River Valleys CoC must operate a coordinated entry system that connects people experiencing homelessness to local housing and services. POSITION SUMMARY

The Coordinated Entry (CE) Manager is the primary role responsible to operate and support ongoing improvement in the region’s Coordinated Entry System (CES). The CE Manager makes and oversees housing referrals, onboards and trains assessors and housing providers in the CES, leads case conferencing, and reports on CE outcomes. The position also coordinates with internal team members to prepare funding requests and consults with the HMIS data system administrator to improve CES data. The CE Manager is part of the River Valleys CoC team, which includes the CoC Director, Performance & Support Manager, Engagement Coordinator, and Housing Navigators.

Supervision: This full-time position will report to the CoC Director and does not directly supervise any staff. This position is a fully remote position. The expectation is that the CE Manager will reside in the twenty-county CoC region or within 30 miles of the border.

ESSENTIAL FUNCTIONS

40% Active CES housing referrals, including:

  • Make CES referrals to housing programs as defined by CoC policies and applicable regulations
  • Maintain active and current CES databases in HMIS and non-HMIS settings to enable quality referrals to housing
  • Lead CES regional case conferencing to increase speed and quality of housing referrals
  • Run and use reports from databases and spreadsheets to track referrals and outcomes

25% CES partner staff training and support, including:

  • Deliver regular training on CES assessor and housing provider procedures
  • Promote best practices in CES, as endorsed by CoC and/or affiliated funding agencies
  • Facilitate orientation and set-up of new CES partners (assessment sites and housing projects) and ensure coordination with HMIS lead for new partners
  • Collect and maintain current CE partner agreements and eligibility information for housing programs and assessors
  • Provide technical support to assessors and housing providers to implement CES procedures in HMIS and non-HMIS settings

25% System improvement, including:

  • Lead CES partners in regular list outreach, data quality review, and data correction
  • Lead Point in Time Count and Housing Inventory data collection by collecting or verifying housing program status and details
  • Generate CE data reports for CoC leadership identify trends, wins, and challenges to improve CES outcomes for people experiencing homelessness
  • Collaborate with the Performance & Support Manager to develop and report on CE-related grants
  • Participate in meetings of the River Valleys CoC, statewide CoCs, and Homeless Management Information System (HMIS) as assigned
  • Staff CoC committees/workgroups as directed by supervisor to complete system improvement tasks

10% Communications & Community Engagement, including:

  • Participate occasionally in county-level homelessness response team meetings to assess and support CE effectiveness
  • Contribute regular CE-related articles for CoC newsletters/blog
  • Maintain current content about CE on CoC website

QUALIFICATIONS

Desired experience and education

  • Providing case management, CES referral management, service coordination, or non- profit customer service/intake
  • Training individuals and groups on specific tasks, procedures, or policies
  • Using CES or other HMIS-based housing referral systems, especially in Minnesota
  • Familiar with homeless response resources in region and best practices in field
  • Working closely with people of many different backgrounds and experiences
  • Communicating publicly about homelessness and response system
  • Writing funding requests for program activities

Critical skills and approaches

  • Highly organized, managing time and tasks proactively to provide prompt service and meet deadlines
  • Maintains confidentiality and follows CES policies and procedures
  • Focuses on goals of CES and position’s role in finding housing solutions for more people
  • Seeks solutions and paths forward in difficult situations
  • Communicates clearly, consistently, and courteously with clients, coworkers, and external partners, verbally and in writing
  • Uses Excel, Outlook, Zoom, and online databases to conduct daily tasks

Other Requirements

  • A valid driver’s license and proof of current automobile insurance
  • Reliable, secure, non-public internet access
  • Dedicated, private workspace to safely complete tasks and store equipment

Travel/Work Schedule

  • Standard business hours, with rare deviation (up to 10 times per year)
  • Occasional travel within the 20-county CoC region
  • Occasional in-state and out-of-state travel for professional conferences/training (less than one week for each occurrence)

Compensation and benefits

All River Valleys CoC staff are employees of the Institute for Community Alliances (ICA), which serves as the lead agency for River Valleys CoC. Compensation and benefits are provided by ICA. The starting salary for this position is $50,000 - $65,000 annually with benefits and travel reimbursement.
