We’re Looking For
A compassionate, dedicated RN Case Manager who makes patients feel at home—whether they’re in their own home, an assisted living facility, or a skilled nursing home. If you’re a hospice or home health Jedi with a valid GA RN license , driver’s license , and at least one year of experience , we want to meet you.
Your Role, Should You Choose to Accept It
Be a steady source of comfort and support to patients and families
Help families understand disease progression, self-care, and nutrition
Collaborate with the interdisciplinary hospice team to create and follow patient care plans
Manage symptoms like a boss and advocate for patient comfort
Maintain timely and accurate clinical documentation
