Job Title: Personal Service Assistant (PSA)
Position Summary
A Personal Service Assistant (PSA) provides non-medical, hands-on support to clients in their home or community setting. The PSA assists individuals with daily living activities to promote independence, safety, and quality of life while following the client’s care plan and all state guidelines.
Key Responsibilities
Personal Care
- Assist with bathing, grooming, oral hygiene, and dressing
- Support toileting, incontinence care, and hygiene needs
- Assist with mobility, transfers, and ambulation (walking support)
Daily Living Support
- Prepare meals and assist with feeding if needed
- Perform light housekeeping (laundry, dishes, sweeping, etc.)
- Help maintain a clean, safe living environment
Companionship & Supervision
- Provide companionship and emotional support
- Monitor client safety and well-being
- Engage clients in appropriate activities
Documentation & Reporting
- Complete accurate daily service notes (EVV if required)
- Report changes in condition, incidents, or concerns to supervisor
- Follow care plan instructions and document tasks performed
Transportation (if applicable)
- Assist with errands, grocery shopping, or appointments
- Follow agency transportation policies
Qualifications
- High school diploma or GED (preferred, may vary by program)
- Must pass background check per Indiana requirements
- Valid driver’s license and insurance (if transporting clients)
- CPR/First Aid certification (preferred or required by employer)

Work Environment
