Insight Partnership Group, LLC is an agency which provides in-home services to our neighbors with an Intellectual Disability, Brain Injury, or Chronic Mental Illness.
Life Skill Site Managers/Direct Support Professional Site Manager is a full-time position. The successful applicant will have experience working with HCBS Waiver or Habilitative Services to individuals in the local community.

ESSENTIAL JOB FUNCTIONS:
· Ability to communicate and work cooperatively with others
· Attend all mandatory in-services and other meetings as required
· Provide direct services according to the person’s IPP schedule in a manner consistent with agency policy & philosophy
· Complete documentation of provided services accurately, timely and within agency policy
· Responsible for scheduling and ensuring client appointments are kept according to schedule and assist clients with bill payments and budget
· Responsible for accuracy of client medication administration including development of the Medication Administration Record
· Complete progress reports
· Responsible for transportation of served individuals as needed
· Complete and submit incident reports
· Ensures proper condition of home and safety procedures
· On call rotation
REQUIRED:
- Valid Driver's License
- Proof of current vehicle insurance
- Background check
