Job Overview
We are seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality, in-home patient care. This role requires traveling to patients’ homes to deliver personalized nursing services, support recovery, and promote independence. The ideal candidate is highly autonomous, organized, and comfortable working one-on-one with patients in a home setting.
Key Responsibilities
- Travel to patients’ homes to provide skilled nursing care and assessments
- Perform comprehensive patient evaluations and develop individualized care plans
- Administer medications, IV therapy, injections, and prescribed treatments
- Monitor patient condition, vital signs, and progress; update care plans as needed
- Provide wound care, dressing changes, and post-surgical care
- Educate patients and families on disease management, medications, and self-care techniques
- Coordinate care with physicians, therapists, and other healthcare professionals
- Maintain accurate and timely clinical documentation and patient records
- Ensure a safe and clean environment within the patient’s home
- Provide emotional support and promote patient independence and quality of life
Required Qualifications
- Active Registered Nurse (RN) license (state-specific)
- Associate’s or Bachelor’s degree in Nursing (ADN or BSN)
- 1–2 years of clinical nursing experience (home health preferred)
- Valid driver’s license and reliable transportation
- CPR/BLS certification
Preferred Qualifications
- Home health, hospice, or case management experience
- Experience with electronic health records (EHR)
- IV therapy and wound care experience
