Job Summary
We are seeking a compassionate and dedicated Case Manager Assistant to join our team. The ideal candidate will be responsible for providing support and guidance to vulberable adults in need, ensuring they receive the necessary resources and services. The Case Manager Assistant supports the Case Manager in facilitating resident care by assisting with resident documentation, coordinating services, and providing administrative support. This role involves ensuring compliance with state regulations, maintaining accurate records, and assisting with the overall coordination of care for residents.
Responsibilities
- Assist the Case Manager in assessing resident care by gathering necessary information, preparing reports, and documenting resident progress.
- Help facilitate interdisciplinary approaches by coordinating services and arranging resources such as transportation and appointments.
- Support the admission process for new residents by assisting with intake documentation, including the Admission Intake Packet, Financial Intake Packet, Resident Consents, Ultimate User Agreement, and Release of Information.
- Schedule assessments and move-in meetings and assist in completing necessary documentation.
- Maintain resident records by reviewing case notes, logging events, and ensuring compliance with relevant regulations.
- Assist in preparing residents for discharge by helping to coordinate discharge and post-discharge requirements and orienting family members.
- Provide administrative support by preparing reports, collecting and summarizing treatment data, and ensuring all documentation is up to date.
- Assist with coordinating and attending medical appointments, completing required medical visit forms, and updating medication lists as needed.
- Help conduct QA reviews of facility and resident supports.
- Assist in the preparation and maintenance of accurate binders according to HCQC requirements.
- Document case management services provided under the direction of the Case Manager.
- Support the Case Manager in conducting quarterly resident review meetings and scheduling new resident meetings with assigned facilities.\
- Coordinate transportation needs for residents as directed by the Case Manager.
- Perform other duties as assigned to support the Case Manager and the overall case management function.
Required Skills

- Proficient with Microsoft Office Suite or related software.
- Strong interpersonal and customer service skills.
- Ability to manage multiple tasks and meet deadlines.
- Familiarity with local resources and programs is preferred.
- Integrity-driven and ability to maintain confidentiality.
Education & Experience
- Associate's Degree or Higher
- 1 years of administrative or customer service experience, preferably in human services.
- Experience supporting individuals with disabilities, mental illnesses, or challenging behaviors is highly preferred.
- Must have a valid in-state driver’s license, provide current registration and insurance, and have a driving record with no more than 2 points within the last 3 years.
- Must be at least 18 years old.
Physical Requirements
- Prolonged periods of sitting at a desk and working on a computer.
- Physically assist Residents getting in and out of vehicles and provide physical assistant at appointments
- Must be able to lift up to 15 pounds at times.

Pay: From $18.00 per hour
