What You’ll Do
- Provide direct patient care, including assessments and care plan development
- Manage a caseload and coordinate care across the clinical team
- Administer medications and treatments per physician orders
- Monitor patient progress and adjust care plans as needed
- Educate patients and caregivers on care and self-management.
- Communicate with physicians and care teams to ensure continuity of care
- Supervise and support HHAs and LPNs
- Complete accurate and timely documentation (OASIS, visit notes, orders)
- Ensure compliance with policies, regulations, and HIPAA standards
- Active RN license
- Graduate of an accredited nursing program (BSN preferred)
- Home health or related behavioral health clinical experience
- Strong communication, organization, and critical thinking skills
- Current CPR certification
- Valid driver’s license and reliable transportation
- Frequent standing, walking, sitting, and use of hands
- Regular bending, kneeling, and reaching
- Occasional lifting and repositioning of patients
- Ability to lift/push up to and over 25 lbs as needed
- Exposure to clinical environments and infectious materials
