Job Summary
The Community Health Advisor is responsible for the delivery of health education to PACE-eligible seniors and/or their families or designated representatives on valuable health care and social services via the PACE model of care.

- Supports Innovative Integrated Health’s (IIH) Business Development department recruitment efforts and enrollment assistance for IIH’s PACE program.
- Accountable for contributing towards department goals, including growing and maintaining patient census
- Participates in community health events, education activities, and PACE-sponsored events, as assigned.
- Develops meaningful, relevant strategic relationships with community organizations including but not limited to, senior and cultural centers, churches, doctors’ offices, and/or housing communities.
- Focus on increasing enrollment leads and tracking conversion rates. Conduct outreach within the PACE designated service area to source marketing outreach opportunities.
- Responsible for tracking all marketing and outreach activity efforts.
- Complete Daily activity reports and submit weekly/monthly data to Director of Outreach/Outreach Manager)
- Engages in mandatory and ongoing training and skill-building activities to effectively achieve intake, enrollment, and program goals.
- Provides PACE referral and community health resource information to PACE-eligible seniors, their families/caregivers, and/or community members in designated service area(s).
- Conducts preliminary enrollment screening interviews to determine eligibility for PACE program services.
- Assists with the set-up of health education and PACE information booths.
- Responsible for adding to and reviewing annual Community Outreach and Marketing calendar to ensure readiness and timely preparation.
- Tracks outreach activity in the Enrollment Activity Tracker
- Submits promotional material requests and reports utilization figures to the VP of Business Development and the Director of Marketing.
- Attend and participate in staff meetings, in-services, projects, and committees as assigned.
- Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance.
- Accept assigned duties in a cooperative manner; and perform all other related duties as assigned.
- Be flexible in the schedule of hours worked.
- May be required to use a personal vehicle for business purposes. If so, valid California Driver’s License, current proof of insurance, and vehicle registration are required.
Knowledge, Skills and Abilities
- Proficient knowledge of computer skills. MS Office (Word, Excel, Access, PowerPoint, and Outlook).
- Knowledge of general office procedures, equipment and filing systems.
- Ability to communicate effectively, both orally and in writing.
- Ability to quickly learn department policies, procedures, goals, and services.
- Skill: Attention to detail and accuracy.
- Ability to change priorities regularly.
- Ability to maintain confidentiality following HIPAA guidelines.
- Ability to communicate clearly and effectively, verbally and in writing with community. members, staff, supervisor and community partners.
- Ability to establish and maintain cooperative working relationship with staff members
- Ability to prepare basic correspondence and simple reports in Microsoft Word.

- Ability to access all areas of the center throughout the workday.
- Ability to lift up to 35 pounds occasionally, 15 pounds frequently, and 7 pounds constantly; required to obtain assistance of another qualified employee when attempting to lift or transfer objects over 25 pounds.
- Requires constant hand grasp and finger dexterity; frequent sitting, standing, walking and repetitive leg and arm movements, occasional bending, reaching forward and overhead; squatting and kneeling.
- Work is generally performed in an indoor, well-lighted, well-ventilated, heated, and air-conditioned environment
Experience
- Minimum of one (1) year working with the frail or elderly population.
- Preferred experience as a customer service, sales, education, training, and/or marketing representative.
Education and Certification(s)
- High School diploma or General Educational Development (GED) certificate or equivalent related work experience (in lieu of Diploma) 3-4 years of direct community outreach experience.
- Valid Driver’s license and auto insurance.
Other Requirements
- All employees facilitating PACE marketing activities are required to pass the PACE marketing exam.
