Position Summary

We are seeking a skilled and compassionate Palliative Care Nurse Practitioner (NP) to join our team. The ideal candidate will provide comprehensive, interdisciplinary care focused on symptom management, goals-of-care discussions, and support for patients facing serious or life-limiting illnesses.

This role offers the opportunity to build meaningful relationships with patients and families while working in a supportive, collaborative environment.

Key Responsibilities

  • Conduct comprehensive patient assessments, including physical, psychosocial, and spiritual needs
  • Develop and implement individualized care plans focused on comfort and quality of life
  • Manage complex symptoms (pain, dyspnea, nausea, anxiety, etc.)
  • Facilitate goals-of-care conversations and advance care planning
  • Collaborate with physicians, nurses, social workers, and interdisciplinary team members
  • Provide education and support to patients and families
  • Coordinate care across settings (home, clinic, hospital, or hospice as applicable)
  • Maintain accurate and timely documentation

Qualifications

  • Current Nurse Practitioner license in Colorado
  • Board certification as a Family, Adult-Gerontology, or Acute Care NP
  • Minimum of 1–2 years of clinical experience (palliative care, hospice, geriatrics, or primary care preferred)
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • DEA license (or ability to obtain)
  • Preferred Qualifications
  • Experience in palliative care or hospice
  • Certification in Hospice and Palliative Nursing (CHPN or ACHPN)
  • Experience with serious illness conversations and end-of-life care
