Position Summary
We are seeking a skilled and compassionate Palliative Care Nurse Practitioner (NP) to join our team. The ideal candidate will provide comprehensive, interdisciplinary care focused on symptom management, goals-of-care discussions, and support for patients facing serious or life-limiting illnesses.
This role offers the opportunity to build meaningful relationships with patients and families while working in a supportive, collaborative environment.
Key Responsibilities
- Conduct comprehensive patient assessments, including physical, psychosocial, and spiritual needs
- Develop and implement individualized care plans focused on comfort and quality of life
- Manage complex symptoms (pain, dyspnea, nausea, anxiety, etc.)
- Facilitate goals-of-care conversations and advance care planning
- Collaborate with physicians, nurses, social workers, and interdisciplinary team members
- Provide education and support to patients and families
- Coordinate care across settings (home, clinic, hospital, or hospice as applicable)
- Maintain accurate and timely documentation
Qualifications
- Current Nurse Practitioner license in Colorado
- Board certification as a Family, Adult-Gerontology, or Acute Care NP
- Minimum of 1–2 years of clinical experience (palliative care, hospice, geriatrics, or primary care preferred)
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- DEA license (or ability to obtain)
- Preferred Qualifications
- Experience in palliative care or hospice
- Certification in Hospice and Palliative Nursing (CHPN or ACHPN)
- Experience with serious illness conversations and end-of-life care
